DD CWS is a joke. It stands for Developmentally Delayed Celebrity Worship Syndrome. There has been talk in the "news"(term used loosely) about CWS. I think that the medical term is really OCD-Obsessive Compulsive Disorder...
I suppose that DD could also stand for Death in Disguise.. again, all in good fun..

Yes. Actually, it is a People Extra issue: "Style Watch" - Fall 2003 (To be displayed on shelves until Sept. 1st).

The first scan is from the first page of the contents and the second scan is from page 62.
